• 1 copy of ANSC 7001 (10-7) w/original signatures and copy of US citizenship document verified.
• 2 FD-258 (REV. 5-11-99) fingerprint cards.
• 1 copy of the New member Exam.
• 1 copy of the B & S course if taken.
• 1 photo on CD or sent by email.
